Remote Patient Monitoring Is Here!
We are excited to announce that 501 Pharmacy is now offering FREE remote patient monitoring! RPM allows us to review your medications, monitor your medical conditions, and coordinate care with your physicians remotely, which helps us gain more insights into your needs and help you navigate the healthcare system so you can receive proper care. If you are 65+ and have Medicare and high blood pressure and/or diabetes, you may qualify for this free service. Keep reading to learn more about all the great program benefits of RPM and how it works!
Free Monitor
Under this program, you will receive a free blood sugar or blood pressure monitor. We set up the device for you and provide instructions on taking your readings at home, which will be transmitted to us right away.
Remote Care
We will remotely monitor your device readings—saving you time and a trip to the doc! We ask patients to get readings from their machines at least every other day, so we get 15 readings per month. If you have a concerning reading, we will check in to see if you might be experiencing any medication-related issues that could be causing high or low readings.
Care Coordination
Upon evaluating your readings, we will consult with your primary care provider on your health and current medications in order to get you the best care. They will receive a monthly report with all your readings and recommendations from us so they are always up to date. So, naturally, doctors love this!
